New referrals are seen at home first to discuss attendance and then offered a place if appropriate. Patients attend one day each week until discharge and an agreed rehabilitation plan is developed which is kept under review by the team.
Patients are able to see the physiotherapist and occupational therapist as often as they need to. Following discharge an effective recall system operates for those with a progressive condition to ensure patients continue to return to the centre as long as they need to, where appropriate.
A social/therapeutic programme of activities helps patients to achieve and maintain both confidence and independence.
How to access the service
To use the service a patient can either be referred by their GP or a consultant, or alternatively they can self-refer by filling out a self-referral form found on the Centre’s website.
