Alert Cards

Our condition-specific alert cards help ensure you get the right care in an emergency. They provide essential information about your condition and practical guidance for healthcare professionals who may not be familiar with muscle wasting conditions.

Order a physical alert card

We offer cards for the following conditions: Becker muscular dystrophy, Charcot-Marie-Tooth disease, congenital muscular dystrophies, Duchenne muscular dystrophy, facioscapulohumeral muscular dystrophy, GNE myopathy, inclusion body myositis, myotonic dystrophy types 1 and 2, oculopharyngeal muscular dystrophy, spinal muscular atrophy types 1, 2, and 3, and for undiagnosed muscle wasting conditions.

We develop our alert cards with input from neuromuscular specialists and our community, making sure the information is accurate, reliable, and genuinely helpful. The card is small enough to carry in a purse or wallet, so it’s always there when you need it.
